Laboratory  test results will be provided directly to you. It is your responsibility to consult with a licensed healthcare provider for any interpretation, evaluation, or guidance regarding your results. ULABSNOW and its affiliated medical professionals do not offer diagnosis, treatment, or interpretation services based on the test results.

 

ULABSNOW, its affiliates, employees, or representatives are not liable for any issues arising from the testing process, including but not limited to errors in specimen collection, delays, misinterpretation, or unreceived results. By using our services, you release ULABSNOW and its representatives from any related claims or liabilities.

 

6. Medical Advice Disclaimer

The laboratory testing offered through ULABSNOW is not a substitute for medical advice, diagnosis, or treatment from a qualified healthcare provider. You accept full responsibility for any decisions or actions taken based on your test results.

 

7. Insurance Reimbursement

You agree not to seek reimbursement from Medicare, Medicaid, Tricare, or any other government-funded insurance program. You acknowledge that you are personally responsible for all charges incurred.

 

8. Testing and Collection Services

Tests ordered through ULABSNOW are performed under the authorization of a licensed physician affiliated with our network. ULABSNOW operates as a collection site; actual laboratory analysis is carried out by third-party laboratories certified under applicable regulations.

 

Test results will be delivered directly to you and, if requested, to a healthcare provider of your choice. Results may be shared via mail, fax, secure digital communication, or in-person pickup.

 

9. Confidentiality and Health Information

All personal health information, including test results, will be handled in accordance with federal and state privacy laws. ULABSNOW will not release your information to third parties without your explicit consent, except where required by law.

 

10. Mandatory Health Reporting
