All purchases made through ULABSNOW, LLC are considered final. All sales are final except where required by law.

 

2. Payment Authorization

You authorize ULABSNOW, LLC  to charge your credit or debit card at the time of order submission.

You represent and warrant that any laboratory test ordered through ULABSNOW is intended solely for your personal use and not for resale, distribution, or use on behalf of any third party, unless expressly permitted by applicable law. You agree to use the services in compliance with all applicable federal and state laws and regulations.

 

4. Age and Residency Requirements

You certify that you are at least 18 years old and a current resident of the United States.

 

5. Test Results and Medical Interpretation

Laboratory test results will be provided directly to you. UlabsNow does not provide medical advice, diagnosis, or treatment. You are solely responsible for consulting with a licensed healthcare provider regarding the interpretation and use of your test results.

All laboratory testing is performed by independent third-party reference laboratories that are appropriately certified under applicable federal and state laws. UlabsNow does not perform laboratory analysis and does not control the testing processes, methodologies, or reporting practices of such laboratories.

 Specimens collected at UlabsNow are submitted to these external laboratories for processing and analysis.

To the fullest extent permitted by applicable law, UlabsNow and its affiliates are not responsible for delays, errors, omissions, or inaccuracies arising from the acts or omissions of third-party laboratories or other service providers involved in the testing process. However, nothing in these Terms shall limit or exclude liability where such limitation is prohibited by law, including liability for gross negligence, willful misconduct, or violations of applicable healthcare regulations.

6. Medical Advice Disclaimer

The laboratory testing services provided through UlabsNow are not a substitute for professional medical advice, diagnosis, or treatment from a qualified healthcare provider. You are encouraged to seek the guidance of a licensed healthcare provider with any questions regarding your test results or medical condition. No physician-patient relationship is created between you and UlabsNow

You acknowledge that any decisions or actions taken based on your test results are made at your own discretion and risk. To the fullest extent permitted by applicable law, UlabsNow disclaims liability for actions taken based on test results without appropriate medical consultation. However, nothing in these Terms shall limit or exclude liability where such limitation is prohibited by law.

7. Insurance Reimbursement

You agree not to seek reimbursement from Medicare, Medicaid, Tricare, or any other government-funded insurance program. You acknowledge that you are personally responsible for all charges incurred.

 

8. Testing and Collection Services

Tests ordered through UlabsNow are performed under the authorization of a licensed physician affiliated with our network. UlabsNow operates as a collection site; actual laboratory analysis is carried out by third-party laboratories certified under applicable regulations.

 

Test results will be delivered directly to you and, if requested, to a healthcare provider of your choice. Results may be shared via mail, fax, secure digital communication, or in-person pickup. 

UlabsNow does not control the operations or testing procedures of such laboratories

 

9. Confidentiality and Health Information

All personal health information, including test results, will be handled in accordance with applicable federal and state privacy laws, including HIPAA where applicable UlabsNow will not release your information to third parties without your explicit consent, except where required by law.

 

10. Mandatory Health Reporting

In compliance with applicable state and local laws, UlabsNow, its partner labs, and affiliated physicians may be required to report certain test results—such as positive tests for communicable diseases or elevated toxin levels—to public health authorities. may be accessed or disclosed as permitted under applicable law and your health plan

 

11. Medical Record Inclusion

Your test results may become part of your permanent medical record. Insurance providers may access these results under the terms of your health plan, if applicable. By using our service, you consent to the inclusion and release of this information as needed for your care.
